The licensed counselor will be responsible for offering support and assistance to patients enrolled in Medication Assisted Treatment for opioid use disorder. Utilizing evidence-based practices and a trauma-informed approach, the counselor will help patients develop individualized coping techniques and symptom management skills that enable then to experience improved quality of life. Through collaboration with doctors, nurses and a multidisciplinary team, the counselor will play a vital role in evaluating patient progress and developing an individualized plan of care.Essential Job Functions:
- Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
- Conducts initial assessment and evaluation to determine eligibility for services.
- Complete Bio-psychosocial assessment.
- Establish individualized treatment plan with each patient and review as clinically indicated.
- Respond, as needed, to patient grievances and complaints.
- Develop aftercare and discharge plans.
- Will provide support to 4 other locations/clinics
- Other related duties as determined by supervisor.
- Must be fully Indiana license as Licensed Clinical Addiction Counselor (LCAC) or Licensed Clinical Social Worker (preferred)
- Previous experience working with chemically dependent individuals.
- Must demonstrate a positive, non-judgmental, and accepting attitude toward our patient population
- Ability to collaborate with compassionate and knowledgeable counseling team members
- Satisfactory criminal background check and drug screen
