Full Job Description
The Social Services Department in Manhattan Division of The New Jewish Home is seeking for a full time Social Worker temp to join their team!
Job Responsibilities:
- Completing MDS and biopsychosocial assessments.
- Facilitate care plan meetings & discharge planning.
- Implement Advanced Directives and End of Life Planning.
- Completing all required documentation in the medical record ( EMR)
- Coordination of care and services including ongoing monitoring of an appropriate and effective person-centered care plan; Strong Clinical, Communication and Organizational Skills
Qualifications:
- Demonstrates judgment of safety and confidentiality issues
- Strong verbal and written communication skills
- Strong customer service focus required
- Knowledge of the community resources
- Social work Long term care experience preferred.
- Master Degree in Social Work, licensed (LMSW)
