ACO Case Manager (RN) Responsibilities:

• As a ACO case manager (RN), you will guide the delegation of responsibilities to the appropriate staff members to ensure proper resident care.
• You will review resident records to become familiar with their history and current circumstances and asses if the Plan of Care to meet resident needs.
• You will collaborate with team members to properly coordinate resident needs.
• You will coordinate the exchange of information before and after a resident is seen.
• You will implement discharge plans in concordance with residents, families, and physicians.

ACO Case Manager (RN) Qualifications:

• Registered Nurse in the State of Illinois.
• No history of abuse or neglect.
• Current CPR certification.
• At least 3 years experience in a skilled nursing setting.
• Experience working with ACOs.
• 1 year of case management, preferred.
